AP Sales Tax box missing

The AP invoice entry sales tax box is missing! I have removed all customizer entries and it is still missing. Any ideas? I am guessing I could recreate it, but I would much rather just get the original back. There are several companies and it is only missing from one of them. Help!

Have you tried resetting the panel or deleting the custom screen and recreating it?

The two most obvious things in my mind are:
1. Did sales tax reporting for A/P get turned off in the setup options for the company in question?
2. Try doing an Update Custom Panels to Current Level (Custom Office | Utilities).

I did. No change. Can't see anything left to change or delete.

Keep in mind there are different options for sales tax in AP Options.
On the "1. Main" panel there is a check box for "Sales Tax Reporting", but to see the tax fields in data entry, you also have to check the box on the "3. Entry" panel, labelled "Calculate Sales Tax in A/P Data Entry".
