When converting forms in a 4.3 to 4.5 upgrade, the forms that have been converted are appearing multiple times in the form code lookup list.

This has happened in Sales Order Printing, S/O Invoice Printing, and A/R Invoice Printing.

Any ideas on what's causing the problem?

But in version 4.50.  Sage doesn't consider it a high priority so it won't be fixed until version 5.0.
