941 report files not present after installing Q2 payroll update

V4.05.  After installing the Q2 payroll update and following the post install instructions, I find that the 941 report files (PR941A.rpt and PRSCHB.rpt) are missing.

 

Any ideas?

Have you tried to print them and anwer yes to "the default form will be used"?

Yes.  In fact, this "use default form" message comes up 11 times before I get to the report setup dialog box.

Update:

 

I have been thinking it might be a rights problem (even though the installer does not throw an error), so I tried installing again with the server's administrator account.  Now I have PR941A.RPT, but still no PRSCHB.RPT.  Is there any place I can get PRSCHB.RPT for v4.05?  We had no problem with the Q1 report.  I'm really scratching me head over this one.

The 941 Schedule B has not changed from the previous version and was not included in the Q2 941 update.  You should still have a copy of the  PRSCHB.rpt from 2009 which is compatible with the Q2 changes and meets IRS guidelines.

It appears the fields for the exempt employees are removed from payroll after installing Service Update 2.  Verifying the database on the 941 form shows the exempt field cannot be found.

The table used for 941 printing (PR_33Crystal941) contains summary information, the additional columns added for the Q2 941 are below.
